Were the Collins software problems predictable ?
Ok.... this is NOT a philosphical question: it's a computer software question - and of course such problems were predictable to anyone who knew what they were doing in the field: just like the dissaster of the Microsoft OS "Vista" was completely predictable to anyone who knew how to design and debug operating systems - and who was given the time and resources to do so. The software problems you speak of, could have easily been prevented by a qualified person given enough time and enough resources to do so.... something which apparently was either not done, or which was done, but the resulting recommendations were ignored or improperly implemented.
